Es más frecuente en mujeres que en hombres. Suele comenzar a mediana edad, aunque es más común en adultos mayores.
Aparece en cortos periodos de tiempo o en forma de brotes. El curso es variable. Las formas severas pueden durar toda la vida.
No se conoce su causa. Los genes, el ambiente y las hormonas pueden tener un papel importante.
