Los pacientes bajo tratamiento antidepresivo sometidos a un procedimiento quirúrgico son muy numerosos. Por tanto, es necesario realizar un correcto manejo de estos fármacos y conocer sus implicaciones anestésicas para evitar complicaciones intraoperatorias y perioperatorias. En general, los antidepresivos más usados son los ISRS, IRSN y la mirtazapina. La conducta más frecuente es mantener el tratamiento, teniendo en cuenta que pueden aumentar el riesgo de sangrado y la aparición de un síndrome serotoninérgico. Con los ADT y los IMAO el manejo no está tan claro, aunque por lo general el tratamiento con los primeros se mantiene y con los últimos se aconseja suspenderlo.
A great amount of patients going to surgical procedures are taking antidepressant agents. So it is needed to know how this affects the anesthetic plan and to make a correct management of these drugs during the perioperative period in order to avoid any complications. Most used antidepressants are SSRIs and SNRI agents. Treatment with this kind of drugs is generally continued but we must be concerned about the risk of both bleeding and serotonin syndrome. The management of tricyclic antidepressants and MAO inhibitors is not so clear. Usually, the decision consists of continued treatment with tricyclic and withholding treatment with MAO inhibitors.
