Spinal dysraphism comprises congenital anomalies characterized by incomplete fusion of the neural tube without exposure of the lesion to the outside. Its main clinical syndrome is the anchoring of the conus medullaris that leads to important neurological manifestations, in many cases irreversible. Given the common ectodermal origin of skin and nervous system, combined malformations of both are common. Various skin stigmas are associated with occult spinal dysraphism, being in multiple occasions the only initial manifestation of the malformation. Magnetic resonance imaging is the fundamental test for the diagnosis of certainty. Greater awareness of this pathology and the recognition of skin markers by professionals would make possible an early diagnosis and individualized management that would avoid its possible sequelae.
