We present a clinical case of a middle age woman without any treatment, who had not previous trauma or surgical intervention. She was attended to in our hospital dispatched from a second level hospital. The pathology she presented was abdominal pain and sensation of mass in her abdomen. In the imaging test we could see the cause of her pain, an spontaneous retroperitoneal hematoma. Reviewing this entity, we found similar clinical characteristics with our patient. But the most important of all this is the treatment, we must keep stable vital signs, knowing the management is mostlyconservative, and be prepared for the surgical procedure, unusual but vital if the situation rapidly worsens.
