La tenosinovitis es una inflamación del tendón y de la vaina que lo recubre1. Las causas de la tenosinovitis del tendón tibial posterior (TTTP) son variables, siendo las más habituales el exceso de uso o sobrecargas bruscas2, pero también se puede producir en el contexto de artritis reumatoide1, enfermedad de Whipple3, enfermedad celíaca (incluso aunque el paciente no haya desarrollado síntomas gastrointestinales4) y como manifestación extra-articular de la gota5. De dejar sin tratamiento, puede progresar hasta la degeneración y rotura del tendón1, 6.
El caso que se describe es el de un varón de 34 años al que, por mala evolución en un plazo de 7 meses, se le realizó tenosinovectomía abierta con revisión de flexor digitorum longus y ligamento Spring del pie derecho. Las muestras remitidas a anatomía patológica revelan una tenosinovitis inespecífica crónica fibrosante con cambios degenerativos basófilos pseudomixoides.
Tenosynovitis is the inflammation of both the tendon and its seath1. Causes of posterior tibial tendon tenosynovitis (PTTT) are variable, being the most usual overuse or sudden overcharges2, but also can be produced in the context of rheumatoid arthritis1, Whipple’s disease3, celiac disease (even without the patient developing gastrointestinal symptoms4) and as an extra-articular symptom of gout5. If left untreated, it can progress to degeneration and tendon rupture1, 6.
The case described is the one of a male (aged 34) who, having a poor evolution during 7 months, underwent an open tenosynovectomy with a revision of the flexor digitorum longus and Spring ligament of the right foot. The sample analysis revealed a nonspecific chronic fibrosing tenosynovitis with basophilic pseudomyxoid degenerative changes.
