El dolor es un problema muy frecuente en los pacientes oncológicos, con una prevalencia del 90% en estadios avanzados. Los analgésicos opioides potentes representan una opción terapéutica fundamental en el manejo del dolor moderado-severo siendo la opción terapéutica que recomienda la OMS, situando estos fármacos en el tercer escalón de “la escalera analgésica de la OMS” con la característica general de no tener techo terapéutico.
OBJETIVO: El objetivo general de este trabajo es analizar mediante una revisión bibliográfica los tratamientos con opioides para tratar el dolor en pacientes con cáncer avanzado.
METODOLOGÍA: Este trabajo se ha desarrollado a través de una revisión bibliográfica de documentos relacionados con el dolor y su tratamiento con opioides. Se han utilizado bases de datos de carácter científico como Cochrane, Pubmed, Scielo, Medline y revistas científicas nacionales e internacionales como Pain, Revista de la Sociedad Española del Dolor, The Clinical Journal of Pain, etc.
RESULTADOS: Es el estudio de los procesos por los que se somete un fármaco a través de su paso por el organismo. Desde su administración hasta su eliminación total del cuerpo. Las características farmacocinéticas varían según cual sea su vía de administración.
CONCLUSIÓN: Entre los opioides débiles, tramadol y codeína, no existen estudios que muestran mayor eficacia de uno frente a otro en el alivio del dolor. Respecto a los efectos adversos se habla de un porcentaje mayor de efectos adversos del tramadol. El coste de Tramadol es 5 veces mayor que al de codeína.
