En las intervenciones quirúrgicas orales en las cirugías maxilofaciales, se precisa un campo quirúrgico libre y por ello la intubación nasotraqueal supone el abordaje de elección si no existe contraindicación para el abordaje de la vía aérea. Además, en este tipo de cirugías, dado que el campo de trabajo se da en la vía aérea, y este campo de trabajo se comparte con el anestesiólogo, pueden darse complicaciones en este, como la edematización, complicando la extubación de los pacientes. Presentamos el caso de una mujer de 86 años que precisa cirugía maxilofacial por cáncer de suelo de boca y su manejo anestésico en dicho caso.
In oral surgical interventions in maxillofacial surgeries, a free surgical field is required and therefore nasotracheal intubation is the approach of choice if there is no contraindication for approaching the airway. In addition, in this type of surgery, given that the field of work is in the airway, and this field of work is shared with the anesthesiologist, complications can occur in the anesthesiologist, such as swelling, complicating the extubation of patients. We present the case of an 86-year-old woman who required maxillofacial surgery for cancer of the floor of the mouth and her anesthetic management in this case.
