Estados Unidos de América es uno de los países desarrollados más ricos del mundo, sin embargo, es uno de los pocos países desarrollados sin una cobertura sanitaria universal. Su modelo sanitario es el libre mercado donde la sanidad está gestionada por empresas privadas y compañías de seguros. Actualmente 45 millones de personas carecen de alguna prestación sanitaria. El objetivo de este trabajo es contribuir a la difusión del modelo sanitario estadounidense con el fin de ofrecer al lector las diferentes ventajas y desventajas del libre mercado en la salud pública. Para ello, se realizó una búsqueda de la literatura (Pubmed, Science Direct) y se consultó páginas oficiales como la Organización de Naciones Unidas, la Organización Panamericana de la Salud y el Ministerio de Sanidad norteamericano. Como resultado se obtuvieron 67 publicaciones de texto completo entre las que se seleccionaron las fuentes con información más relevante en política sanitaria, salud pública, gestión y desarrollo del modelo sanitario estadounidense en las dos últimas décadas. Las conclusiones han sido que la cobertura sanitaria universal está aún muy lejos de conseguirse en los Estados Unidos, la falta de una red de atención pública junto a todas las organizaciones privadas que deben gestionar el dinero hacen que no llegue realmente a los más pobres. Su sistema está orientado hacia la innovación mundial en investigación dejando a un lado la atención comunitaria.
The United States of America is one of the richest developed countries in the world, yet it is one of the few developed countries without universal health coverage. Its healthcare model is the free market where healthcare is managed by private companies and insurance companies. Currently 45 million people have any health benefits. The objective of this work is to contribute to the broadcast of the American health model in order to offer the reader the different advantages and disadvantages of the free market in public health. To do this, a literature search was carried out (Pubmed, Science Direct) and official pages such as the United Nations Organization, the Pan American Health Organization and the North American Ministry of Health were consulted. As a result, 67 full-text publications were obtained from which the sources with the most relevant information on health policy, public health, management and development of the US health model in the last two decades were selected. The conclusions have been that universal health coverage is still far from being achieved in the United States, the lack of a public care network along with all the private organizations that must manage the money mean that it does not really reach the poorest. Its system is geared towards global innovation in research leaving aside community care.
