Introducción: El ductus arteriovenoso es vital en la fase fetal y se cierra de forma fisiológica tras el nacimiento. Los niños pretérmino tienen más probabilidad de mantener el ductus arteriovenoso permeable, con influencia de varios factores de riesgo. Para su cierre se emplea tratamiento farmacológico y quirúrgico.
Objetivos: Identificar las medidas empleadas para el manejo del ductus arteriovenoso permeable en niños pretérmino ingresados en una unidad de cuidados intensivos.
Metodología: Revisión de la bibliografía en las bases de datos: PUBMED, CUIDENPLUS, COCHRANE y los metabuscadores Google Académico y Alcorze. Se combinan los descriptores “Patent ductus arteriosus”, “Premature infant”, “Treatment” y “Care intensive unit”, con el operador booleano AND. Se seleccionaron publicaciones en castellano, inglés y portugués publicadas a partir del 2015.
Resultados: Para el cierre del ductus arteriovenoso permeable se recomienda iniciar un tratamiento farmacológico con Indamacina o Ibuprofeno, desplazando el empleo de Paracetamol. Como último recurso se recurre al cierre quirúrgico. Entre las intervenciones de enfermería destacan la monitorización de constantes, control de signos y síntomas, administración de tratamiento y atención a la familia.
Conclusiones: existen dos opciones de tratamiento para el cierre del ductus arteriovenoso permeable: farmacológico y quirúrgico. Las intervenciones enfermeras son individualizadas y holísticas, dirigidas al manejo y bienestar tanto del niño pretérmino como de su familia.
Introduction: The ductus arteriovenosus is vital in the foetal phase and closes physiologically after birth. Preterm infants are more likely to keep the ductus arteriovenosus permeable, influenced by several risk factors. Pharmacological and surgical treatment is used to close it.
Objectives: To identify the measures used for the management of ductus arteriosus in preterm infants admitted to an intensive care unit.
Methodology: Review of the literature in the databases: PUBMED, CUIDENPLUS, COCHRANE and the meta-searches Google Academic and Alcorze. The descriptors “Patent ductus arteriosus”, “Premature infant”, “Treatment” and “Care intensive unit” are combined with the Boolean operator AND. Publications in Spanish, English and Portuguese published from 2015 onwards were selected.
Results: To close the ductus arteriosus it is recommended to start a pharmacological treatment with Indamacin or Ibuprofen, replacing the use of Paracetamol. As a last resort, surgical closure is used. Nursing interventions include monitoring of vital signs and symptoms, administration of treatment and care of the family.
Conclusions: There are two treatment options for the closure of the patent ductus arteriosus: pharmacological and surgical. Nursing interventions are individualized and holistic, aimed at the management and well-being of both the preterm child and his/her family.
