The G20210A mutation in the prothrombin gene is among the best known hereditary thrombophilic disturbances, but its appearance in homozygosis is extremely rare.
We present an unusual case of a patient that succeeded to complete her first and unique pregnancy through assisted reproduction with in vitro fertilization and embryo transfer. No anticoagulant prophylaxis was done during the process, and the patient was successful to get oocyte retrieval, embryo transfer, implantation and posterior pregnancy without any complication.
Her thrombophilic risk was evaluated years later, upon a new diagnosis of neoplastic disease, and she was found to have an homozygous G20210A mutation in the prothrombin gene.
We conclude than the success and absence of complications in a previous procedure of assisted reproduction do not allow to discard a suspicion on the potential existence of this mutation, even in homozygosis.
