Polymyalgia rheumatica (PMR) is an inflammatory disease with acute onset, with a higher incidence in older adults, characterized by pain and stiffness in the shoulder girdle and pelvis. Its diagnosis can be complex due to clinical nonspecificity, the absence of pathognomonic signs, and the existence of numerous differential diagnoses. We present the case of a 63-year-old man with long-standing joint pain and neck pain, accompanied by progressive functional limitation, in whom various musculoskeletal and rheumatological causes were considered. Finally, the clinical picture, the favorable response to corticosteroids, and the support of the literature allowed us to confirm the diagnosis of PMR. In relation to this case, we review the main diagnostic and therapeutic difficulties described in the recent literature, highlighting the importance of a multidisciplinary approach and the use of imaging techniques.
