El manejo de enfermedades crónicas representa un reto creciente para los sistemas de salud, exigiendo cuidados continuos que involucren activamente a pacientes y cuidadores. Las Tecnologías de la Información y la Comunicación (TIC) emergen como herramientas estratégicas para fortalecer el soporte social, la educación sanitaria y la autogestión. La enfermería comunitaria, debido a su proximidad al paciente y su entorno, posee un rol clave en la integración efectiva de las TIC. Este artículo revisa las aplicaciones prácticas de las TIC en el manejo de enfermedades crónicas, identifica barreras como la brecha digital y la protección de datos, y destaca el papel fundamental de la enfermería comunitaria para garantizar su adopción significativa. Se proponen recomendaciones para potenciar la transformación digital de los cuidados.
The management of chronic diseases represents a growing challenge for healthcare systems, requiring continuous care that actively involves patients and caregivers. Information and Communication Technologies (ICT) emerge as strategic tools to strengthen social support, health education, and self-management. Community nursing, due to its proximity to patients and their environment, plays a key role in effectively integrating ICT. This article reviews the practical applications of ICT in chronic disease management, identifies barriers such as the digital divide and data protection, and highlights the fundamental role of community nursing to ensure meaningful adoption. Recommendations are proposed to enhance the digital transformation of care.
