Presentamos el caso de un varón de 20 años que acude a urgencias por odinofagia y sensación de molestia cervical de 24 horas de evolución. El paciente se encuentra afebril y sin síntomas respiratorios. La radiografía de tórax revela neumomediastino, confirmado posteriormente por tomografía computarizada (TC), sin evidencia clara de causa subyacente. Se realiza un manejo conservador con buena evolución clínica. Este caso destaca la importancia de considerar el neumomediastino espontáneo (NME) en el diagnóstico diferencial de síntomas cervicales inespecíficos, incluso en ausencia de antecedentes respiratorios o traumáticos.
We present the case of a 20-year-old male who goes to the emergency room for odinophagy and sensation of cervical discomfort from 24 hours of evolution. The patient is afebrile and without respiratory symptoms. Chest X-ray reveals pneumomediastinum, later confirmed by computed tomography (CT), with no clear evidence of underlying cause. Conservative management is performed with good clinical outcome. This case highlights the importance of considering spontaneous pneumomediastinum (SMN) in the differential diagnosis of nonspecific cervical symptoms, even in the absence of a respiratory or traumatic history.
