La hepatitis autoinmune es una enfermedad inflamatoria crónica del hígado que afecta predominantemente a mujeres y puede iniciarse a cualquier edad. Se caracteriza por autoanticuerpos circulantes y niveles elevados de globulina sérica. La enfermedad puede comenzar como hepatitis aguda y avanzar a hepatitis crónica y cirrosis.
La presentación clínica es variada, desde asintomática hasta síntomas graves o insuficiencia hepática aguda. El diagnóstico se basa en hallazgos serológicos e histológicos, así como en la exclusión de otras enfermedades hepáticas. En adultos con síntomas compatibles, se realizan pruebas de anticuerpos como ANA, ASMA, AMA, anti-LKM-1 y niveles de IgG o gammaglobulina. Si estos son negativos, se buscan anticuerpos adicionales como anti-SLA/LP y anti-actina. La biopsia hepática es preferible para confirmar el diagnóstico y guiar el tratamiento, especialmente en casos con hallazgos atípicos o autoanticuerpos negativos. El tratamiento inicial típicamente incluye glucocorticoides, con o sin azatioprina o 6-mercaptopurina.
