Paciente varón de 53 años con traumatismo en ambos MMII, desarrolló placas necróticas tras infección, atendido por cirugía plástica. Las lesiones fueron tratadas durante dos meses en el centro de salud, presentando infecciones recurrentes que requirieron ajustes en el tratamiento antibiótico. Con un peso de 110 kg y altura de 170 cm, tiene un IMC de 38 (obesidad clase 1). Neurológicamente estable y semi-dependiente para actividades básicas debido a patología vascular, presenta MMII edematizados con mala perfusión tisular. Las úlceras vasculares en ambas piernas, con secreción purulenta, mostraron infección por pseudomona aeruginosa multirresistente. Se pautó amoxicilina + ácido clavulánico y, tras ingreso hospitalario para antibioterapia precisa, la cita para desbridamiento e injerto fue retrasada 3 meses para tratar la infección.
A 53-year-old male patient with trauma to both lower limbs developed necrotic plaques after an infection and was treated by plastic surgery. The lesions were treated for two months at the health center, presenting recurrent infections that required adjustments in antibiotic treatment. Weighing 110 kg and 170 cm tall, he has a BMI of 38 (class 1 obesity). Neurologically stable and semi-dependent for basic activities due to vascular pathology, he presents with edematous lower limbs with poor tissue perfusion. The vascular ulcers on both legs, with purulent secretion, showed infection by multidrug-resistant Pseudomonas aeruginosa. Amoxicillin + clavulanic acid was prescribed, and after hospital admission for precise antibiotic therapy, the appointment for debridement and grafting was delayed by 3 months to treat the infection.
