Se ha realizado una Valoración Geriátrica Integral (VGI) de un caso clínico concreto desde Atención Primaria que considera a la paciente geriátrica de forma multidisciplinaria y multidimensional.
La VGI basa su método en la recopilación de información del paciente a partir de la historia clínica previa y actual, anamnesis, exploración física y mediante el uso de herramientas específicas como escalas e índices validados que proporcionan una visión holística del mismo pudiendo identificar problemas a nivel físico, psíquico, social y funcional. La atención dada al paciente geriátrico a través de la VGI facilita garantizar un plan de atención adecuada, efectiva y de calidad.
El objetivo principal de esta valoración es diseñar un plan de atención individualizado preventivo, terapéutico y rehabilitador, con el fin de permitir a la paciente alcanzar su mayor nivel de autonomía posible, así como una buena calidad de vida.

The main objective of this evaluation is to design an individualized preventive, therapeutic and rehabilitative care plan, in order to allow the patient to achieve the highest possible level of autonomy as well as a good quality of life.
